Creating Consent Forms

Create consent forms such as HIPAA consent, Opioid agreements, Medical Marijuana, and other consent forms that require a signature every set number of days.

To create a form, follow the steps below:

  1. Go to Setup.
  2. Click Consent Forms.
  3. Click Add Consent Form.
  4. Enter a title and body text for the form.
  5. Optional: Set if and when the consent expires.
  6. Click Save.
