Creating Surveys

Included with Digital 360 Health Office Module

Easily collect valuable patient data using validated surveys.

To create a survey, follow the steps below:

  1. Go to Setup.
  2. Click Surveys.
  3. Click Add Survey.
  4. Enter in a title and select the survey category.
  5. Optional: Customize survey by requiring reviews, capturing metrics, adding resources, consent forms, etc.
  6. Click Save.
  7. Click the Questions tab.
  8. Select one or multiple questions, then click Attach Question(s) or Add New Survey Question.

Optional: Click the Configured Scores tab to create a total, average, or percentage score based on the following question types:

  • 0 or 1 Rating
  • 0 to 2 Rating
  • 0 to 3 Rating
  • 0 to 4 Rating
  • 0 to 5 Rating
  • 0 to 6 Rating
  • 0 to 10 Rating
